Correct a death certificate in India

For a wrong name, age or demographic detail, request correction from the Registrar holding the existing death record. The application route and evidence depend on the state, local authority and type of error.

Death certificate correction: 36 state and UT procedures

Select the place where the death was registered, not where the family now lives. Every entry below separates a confirmed correction service or legal rule from a Registrar-led action where public instructions are incomplete. Government PDF references are marked as forms, manuals or rules—not mislabelled as application links.

Andhra PradeshMeeSeva / CDMA municipal correction

Online / assisted correction listed

Step-by-step procedure

  1. Check the original certificate for the Registration Unit ID, registration year and registration number; the official kiosk manual also allows a record search when these are missing.
  2. At a MeeSeva centre ask for the CDMA service “Corrections in Death Certificate”, not a fresh certificate or a delayed registration. The published instructions are for a kiosk operator.
  3. Have the operator retrieve the death entry, select the individual fields marked for correction and enter their replacement particulars. Confirm every value before submission.
  4. Provide the application and evidence for the error; where a death was medico-legal, ask about police records, FIR and post-mortem report, as identified in the CDMA manual.
  5. Obtain the service acknowledgement/payment receipt. After municipal scrutiny and approval, obtain a fresh extract and verify its details.

Documents and proof: The kiosk manual names the application form and supporting evidence such as the original certificate, identity/education records, hospital letter and, where relevant, an affidavit or medico-legal documents.

Important limit: The linked instructions are an official operator manual, not a public self-service application. The CDMA route covers municipal records; check village/Panchayat registrations with the local Registrar. Old manual charges should not be represented as current fees.

Arunachal PradeshItanagar CRS transition / Registrar elsewhere

Check online scope; Registrar may be required

Step-by-step procedure

  1. Identify the issuing unit; a municipal certificate from Itanagar is not handled through the old IMC birth/death submission form.
  2. For an Itanagar Municipal Corporation record, use the Central Civil Registration System route identified in IMC’s public notice or visit the IMC office for correction assistance.
  3. Locate the existing registration number, take the issued certificate and proof of the correct entry, and explain exactly what was recorded wrongly.
  4. Ask the Registrar/municipal officer whether the change is entered by staff through CRS and what authenticated proof is required for that field.
  5. Collect the acknowledgement; after processing, obtain and compare the replacement extract against the original entry.

Documents and proof: Existing certificate, registration particulars and documentary evidence of the precise error; the IMC notice does not publish a field-by-field checklist.

Important limit: The IMC notice is specific to Itanagar. Other districts must confirm their record-holding Registrar; a CRS homepage is not proof of a citizen-accessible correction button.

AssamDHS / urban and hospital Registrars

Registrar / manual confirmation

Step-by-step procedure

  1. First identify whether the death was registered by a hospital Registrar, a municipal registration unit such as Guwahati Municipal Corporation, or the relevant rural unit. Assam government guidance identifies institutional, municipal and local registration units.
  2. Obtain the original record number or certificate from that unit. Do not select “Apply for Death Certificate” as a correction request: Assam’s public issuance directions concern fresh certificates.
  3. Write to the Registrar under Section 15 of the RBD Act stating the present entry, the correct value, why it was entered wrongly and which supporting record proves it.
  4. Ask the Registrar whether the Assam rules require witness declarations for this kind of substantive error or only a record-level enquiry for a clerical mistake. Provide hospital verification if the hospital supplied the wrong data.
  5. Request a dated acknowledgement and written disposal. After approval, ask for a corrected extract; if refused, obtain the order for appeal.

Documents and proof: Original death entry/certificate; hospital report for institutional errors; older identity or family records proving the disputed detail; declarations only if required by the Registrar.

Important limit: Assam’s published registration pages identify the competent Registrar, but are mostly about ISSUANCE, not death-certificate correction; no usable statewide online correction link or published correction-only checklist could be verified. Issuance fees/timelines on other Assam pages are not correction fees/timelines.

BiharOriginal Registrar; RTPS is issuance only

Registrar / manual confirmation

Step-by-step procedure

  1. Locate the registration office and the exact death-record number. Bihar RTPS lists issuance through counter/CRS but does not show a separate statewide correction service in its public menu.
  2. Submit a signed request to the Registrar maintaining that entry; specify whether the issue is spelling/clerical or a substantive factual difference.
  3. Attach the original certificate and contemporaneous supporting records. For a substantive change ask what declarations or witnesses are required under the applicable Bihar RBD Rules.
  4. If the record is held in an older register, request a manual register search rather than making a fresh death-registration application.
  5. Receive the correction decision and revised certificate; if the Registrar rejects the request, request written reasons and follow the statutory appeal route.

Documents and proof: Existing certificate or entry search, supporting records proving the desired correction, applicant’s ID and any declarations requested under the applicable rules.

Important limit: The RTPS source is an ISSUANCE directory, not a correction application. A complete public Bihar correction form, fee and service time were not independently confirmed.

Chhattisgarhe-District death-certificate correction listing

Check online scope; Registrar may be required

Step-by-step procedure

  1. The Chhattisgarh e-District catalogue names “Death Certificate Correction” under Planning, Economics and Statistics. Find the concerned registration office using the certificate.
  2. Contact the local Lok Seva Kendra / Registrar and confirm whether the production portal exposes the same service; do not upload personal papers to any UAT or test address.
  3. Ask the centre to select the correction service, identify the existing death entry and provide the original certificate and evidence for the replacement particulars.
  4. Obtain the application ID and track the request through the actual state production portal or service centre.
  5. After the competent authority’s check, collect the amended extract and confirm the registration number and corrected fields.

Documents and proof: An e-District instruction page identifies a mandatory “Choice Certificate” enclosure but does not clearly explain what this means for different errors; obtain the current centre checklist before preparing declarations.

Important limit: An official UAT/test page lists the service, a 7-day period and a ₹30 fee, but it is NOT safe to use as a live filing link or to assert its settings apply to the production system.

GoaGoa Online: legacy records pre-1970

Online / assisted correction listed

Step-by-step procedure

  1. Check the year of the original registration. Goa Online’s Registration Department correction application expressly covers records prior to 1970.
  2. For a qualifying old record, create/sign into Goa Online and open Registration Department → Correction in Death Certificate; record the existing and proposed values.
  3. Attach evidence of the incorrect entry, submit, and save the acknowledgement for status tracking.
  4. After SMS/email asking for verification, visit the concerned Civil Registrar-cum-Sub-Registrar headquarters with the ORIGINAL proof documents; this physical verification is part of the published process.
  5. Following the department’s scrutiny and decision, obtain the corrected certificate. For deaths registered from 1970 onwards, approach the Municipal Council/Village Panchayat Registrar holding the entry instead of assuming this legacy service applies.

Documents and proof: The official Goa form asks for applicant and death-registration particulars, present and proposed names/date and relationship to the deceased; proof of error is submitted and original records produced on intimation.

Important limit: Important: the verified online correction service explicitly says it is exclusively for registrations prior to 1970. A separate “Death Certificate (1971–till date)” service is for certificates, NOT proof of a modern correction application.

GujaratCity / town / village Registrar

Registrar / manual confirmation

Step-by-step procedure

  1. Find the jurisdiction shown on the death record (municipal corporation, nagarpalika or rural Registrar); Gujarat does not have one verified public amendment form for all local bodies.
  2. For a city record, contact the local birth-and-death registration department. Surat’s online death certificate tools provide search, print and verification, NOT an advertised correction screen.
  3. Present the old certificate and a written comparison of incorrect versus correct details, with identity or source records showing the correct data.
  4. Ask the Registrar to classify the error under Section 15 and the applicable Gujarat Rules and explain whether a declaration, hospital confirmation or higher approval is needed.
  5. Follow up with that registration office for the approved amendment and new extract; verify it in the local certificate system where supported.

Documents and proof: Original registration reference, proof linked to the field being changed, and a hospital record if the error originated in an institution.

Important limit: Do not mistake Surat’s online death certificate download/verification for a death-correction facility. No single Gujarat-wide online correction workflow or fixed fee was verified.

HaryanaSARAL birth/death record correction

Online / assisted correction listed

Step-by-step procedure

  1. The Urban Local Bodies Department expressly lists “Application for Correction in Birth or Death Record” under Birth & Death Certificate services.
  2. Visit SARAL Haryana or its assisted service centre and find the correction option; identify the urban authority that issued the death record.
  3. Use the old certificate/registration particulars to specify the incorrect field; supply documents supporting the corrected name, date or address as directed by the application.
  4. Submit through the correct registered service, retain the SARAL application reference and answer any document-deficiency notice.
  5. After registrar approval, collect or download the corrected record as the local authority permits.

Documents and proof: Exact enclosures differ by field; obtain the live SARAL correction service checklist before submission.

Important limit: The ULB page establishes a correction service but does not publish a universal statewide field-by-field document list or completion time. Do not rely on unrelated child-name manuals.

Himachal PradeshLocal Registrar; CRS staff workflow

Registrar / manual confirmation

Step-by-step procedure

  1. Identify the Registrar who originally recorded the event; the HP Civil Registration guidance distinguishes local institutions and rural/urban registration units.
  2. Bring the issued certificate to that unit and request a check of the original death-register entry, especially if the record predates digitisation.
  3. Explain the erroneous particular and furnish the supporting contemporaneous record; if it is an institutional error, request confirmation from the medical institution that reported the event.
  4. Ask the Registrar to process correction under Section 15 and the current HP registration rules; obtain the acknowledgement and any additional document request in writing.
  5. Get the revised certified extract after approval, or a written refusal identifying the appellate authority.

Documents and proof: Certificate/registration number; correct identity or medical reporting record and applicant identification. Detailed correction enclosures vary.

Important limit: Government HP guidance expressly recognises correction of death-record errors under Section 15, but no verified public citizen self-service correction screen or statewide completion time was located.

JharkhandRecord-holding Registrar, not JharSewa issuance

Registrar / manual confirmation

Step-by-step procedure

  1. Read the issuing office on the certificate. JharSewa publicly lists “Issue of Death Certificate” through a kiosk, not a confirmed service for changing an existing entry.
  2. Ask that issuing Registrar for the written Section 15 correction procedure and how an older manual record can be located.
  3. Submit a correction representation quoting the existing registration number, the wrong entry and the verified replacement value.
  4. Supply supporting evidence and any register-level declarations the Registrar requires; do not attach documents solely because JharSewa lists them for initial issuance.
  5. Obtain the authority’s acknowledgement and order; request the updated extract or take a refusal to the District Registrar.

Documents and proof: Existing record, affected identity/relationship/medical documents, applicant identification.

Important limit: The published JharSewa 30-day service and its death-at-home/hospital checklist are for ISSUANCE; they are not correction requirements. A direct correction portal was not verified.

KarnatakaeJanMa correction at operator / urban local Registrar

Check online scope; Registrar may be required

Step-by-step procedure

  1. Distinguish rural eJanMa registration from municipal records (including Bengaluru), using the office printed on the certificate; do not automatically direct Bengaluru urban cases to a rural Nada Kacheri.
  2. For a rural eJanMa record, take the old certificate to the concerned Nada Kacheri data-entry operator; official district guidance directs corrections there.
  3. The operator’s eJanMa manual says to locate the death registration number using “Death Search” if necessary, then use Deaths → Death Correction and enter the existing registration number.
  4. Provide the supporting proof, confirm the revised details and ask for the submission receipt; the record proceeds to the approving authority.
  5. Once approved, collect the revised certificate and verify the exact date, name and parent/spouse fields. For municipal records, apply to the corporation/city Registrar that holds the register.

Documents and proof: Original death certificate, registration number or search details, evidence for the correct value and any source-institution clarification requested by the registrar.

Important limit: The eJanMa manual describes a STAFF interface, not a guaranteed citizen login. Its screenshots are old and the workflow may have changed. Urban and rural authorities differ.

KeralaLocal self-government body / KSMART

Online / assisted correction listed

Step-by-step procedure

  1. Check which Kerala local government registered the death (Panchayat, Municipality or Corporation). The Kerala e-services dashboard explicitly lists “Death – Application for Corrections”.
  2. Use the current citizen service route available to that local body—KSMART or its designated local front office/Akshaya channel—and select an EXISTING death-record correction, not new death registration.
  3. Enter the certificate or entry details and the correction requested. Attach supporting evidence for the field; if online submission is unavailable, file at the registration unit’s front office.
  4. Retain the application number and watch for local-body scrutiny or requests for additional documents.
  5. Once the Registrar approves, retrieve the corrected certificate and compare it with the earlier version.

Documents and proof: Prior death certificate/number, applicant identification and proof of the proposed change; local body may specify additional evidence.

Important limit: Kerala official pages have transitional information: the state dashboard confirms a correction service but the LSGD information page still refers to ILGMS for Gram Panchayats. Verify the active channel with the registering local body rather than relying on an old ILGMS link.

Madhya PradeshMunicipal / rural Registrar; no verified correction screen

Registrar / manual confirmation

Step-by-step procedure

  1. Locate the entry-holding municipal corporation, municipality or rural Registrar. A state general e-service landing page alone cannot correct a death record.
  2. Request a search of the original register using the certificate number and date; if the original was a hospital-reported death, identify the institution that supplied the wrong information.
  3. File a Section 15 application to the Registrar specifying both versions of each disputed field and attach the original extract plus documentary proof.
  4. Ask what the MP rules require for a clerical change versus a substantive dispute and whether the old register needs higher-office retrieval.
  5. Keep the acknowledgement and obtain the corrected certified extract or a written refusal for an appeal.

Documents and proof: Certificate/registration number, evidence proving the fact as of the death date, and hospital confirmation when relevant.

Important limit: MP’s general service portal does not establish a dedicated correction application, document checklist, fee or turnaround for all districts.

MaharashtraIssuing municipal Registrar; Solapur has detailed online procedure

Check online scope; Registrar may be required

Step-by-step procedure

  1. Identify which municipal corporation, municipal council or other Registrar issued the certificate. Solapur’s death-registration correction page is a verified local example, not a statewide portal.
  2. For a SOLAPUR record, select “Death registration correction”; the city permits family members to apply. Check the permitted field: surname, parent/spouse detail, address, spelling and certain other particulars are listed.
  3. For general record changes Solapur requires the original certificate, applicant self-declaration, TWO witness self-declarations and their identity documents plus supporting records for the person whose details change.
  4. For spelling-only correction or entry of English text, Solapur instead lists the original certificate plus any TWO specified identity documents. Scan/upload the right set, submit and retain the online acknowledgement.
  5. Collect the revised extract after scrutiny. For Mumbai, Pune and other local authorities ask their own Registrar for the correct method and documents.

Documents and proof: Solapur’s official checklist lists self-declarations/witness IDs for substantive changes and a separate two-document route for spelling/English-entry issues.

Important limit: Solapur publishes a 31-day service table and ₹30 per certificate copy, while another city health FAQ mentions 21 working days; do not present either as Maharashtra-wide or conflate correction and copy fees.

ManipurLocal municipal / rural Registrar; no confirmed correction e-service

Registrar / manual confirmation

Step-by-step procedure

  1. Find whether the death was recorded by a municipality, health institution or local rural Registrar; Manipur public e-services material lists DEATH CERTIFICATE issuance, not an established correction workflow.
  2. Verify the original death registration with the issuing unit and request its correction application under Section 15.
  3. Describe each incorrect detail beside the proposed correct value; attach the existing certificate and evidence dated close to or before the death when appropriate.
  4. Seek the unit’s instructions for a hospital-source correction, declarations or superior-register approval, rather than using an issuance checklist.
  5. Request receipt and written decision; obtain a new extract after approval or escalate under Section 25A.

Documents and proof: Original certificate, correct personal/medical/relationship documents and applicant ID; extra enclosures are office-specific.

Important limit: A Manipur government e-services memorandum identifies death certificate applications only. It does not establish a citizen self-service correction form or fee.

MeghalayaRegistration unit; June 2026 BDO order in specified jurisdiction

Check online scope; Registrar may be required

Step-by-step procedure

  1. Identify the original Health or Urban Affairs registration unit. Meghalaya’s public service catalogue lists separate death certificate issuance services, which cannot be treated as correction forms.
  2. Check district instructions before visiting: a Meghalaya government announcement dated 26 June 2026 says applications for registration, correction, cancellation and related matters are to go to the respective Block Development Offices under the stated administrative order.
  3. Present the existing death entry and a written error-versus-correct detail statement to the office identified for your jurisdiction; ask how it coordinates with the original Registrar.
  4. Provide documentary proof, respond to any enquiry and obtain the recorded decision.
  5. Collect the corrected certified death extract from the holding Registrar after approval.

Documents and proof: Old certificate/entry, field-specific proof, applicant ID; the announcement does not give a complete evidence checklist.

Important limit: The 26 June 2026 BDO direction appears as a specific administrative announcement; its district/territorial scope must be checked before using it statewide. Online Meghalaya issuance is NOT an online correction service.

MizoramRegistrar under Mizoram Rules 2007, Rule 11

Registrar / manual confirmation

Step-by-step procedure

  1. Go to the Registrar who maintains the original death entry and obtain the existing registration details.
  2. For a spelling/clerical error, report the specific problem and ask the Registrar to examine the original register under Rule 11(1) of the Mizoram RBD Rules 2007.
  3. For a substantive error, Rule 11(4) provides for a declaration explaining the error and true facts made by TWO credible people who know those facts; assemble the supporting evidence and ask about the precise format.
  4. If the register is held elsewhere, Rule 11 contemplates requesting it and countersignature; ask the office to handle that step rather than filing a duplicate death.
  5. After a signed correction decision, request the corrected extract; pursue the statutory appeal route for a refusal.

Documents and proof: Existing certificate, field-specific records, and for substantive error the two-credible-person declaration provided for in Rule 11(4).

Important limit: This is a published legal Registrar procedure, NOT a verified online correction form. The 2007 rules should be checked against any later applicable amendments.

NagalandDistrict Registrar; online registration transition 1 June 2026

Registrar / manual confirmation

Step-by-step procedure

  1. Look at the existing certificate to determine its district and registration unit. Do not seek a new registration merely because an older manually issued certificate cannot be found online.
  2. Nagaland’s DIPR announcement states that registrations moved online from 1 June 2026, that old manual certificates remain valid and that digitisation is ongoing.
  3. Take the old record and a signed correction request to the district Registrar or registration unit; distinguish a data-entry mismatch from correction to the official death register.
  4. Ask which staff system or manual-register procedure covers the certificate’s year and what documentary evidence is needed.
  5. Get an acknowledgement, then request the new corrected extract once an authorised correction is recorded.

Documents and proof: Existing manual/digital certificate, registration area/year and proof of the exact field.

Important limit: The Nagaland announcement confirms transition and where to seek help; it does not describe a complete public death-correction form or permanent online citizen workflow.

OdishaOdisha portal Registration Correction; verify issued-record scope

Check online scope; Registrar may be required

Step-by-step procedure

  1. Open the Odisha Birth and Death Registration System citizen services, select Online Death Services and locate the “Registration Correction” option.
  2. The portal describes this feature as correcting a birth/death REGISTRATION FORM via application or registration number, so first determine whether your death record is still an application or has already been approved/certified.
  3. If the application is pending and eligible, provide the existing identifier, select correction and follow the portal form; keep the acknowledgement and track it.
  4. If a certificate is already issued, ask the responsible Registration Unit/Registrar whether this same feature can lawfully amend the final register under Section 15. Do not assume that editing an application edits the registered record.
  5. Submit supporting source records to the Registrar where required and obtain a fresh approved extract; check it after retrieval.

Documents and proof: Registration/application number and supporting documents establishing the original reporting error; the portal does not publish a dependable universal approved-certificate checklist.

Important limit: CRITICAL: “Registration Correction” is officially described as correction of a registration FORM. Its applicability to already-issued certificates has not been established publicly. Avoid claiming a guaranteed end-to-end online death-certificate amendment.

PunjabPunjab correction form / Sewa Kendra

Online / assisted correction listed

Step-by-step procedure

  1. Download the official “Correction in Death Certificate” form from Punjab’s Health & Family Welfare forms directory (the PDF is explicitly named for death correction).
  2. Complete the applicant’s details and relationship to the deceased, registration number, date, office, district and—when digitally signed—the DSN number.
  3. Fill the correction particulars, present entry and proposed entry. Attach the old certificate, self-declaration and category-specific documentary evidence specified in the form; obtain witness declarations/IDs when the form requires them.
  4. Submit through the designated Sewa Kendra/authority and obtain an application number and payment receipt; the official PDF shows government and facilitation fees but they should be rechecked before payment.
  5. Answer scrutiny queries and collect the revised certificate after approval; check every name in Punjabi/English where applicable.

Documents and proof: Original certificate; applicant photo/ID, registration details, self-declaration, field-specific evidence and two credible-person declarations where called for by the published checklist.

Important limit: Official form version is dated 2019/2020; consult the Sewa Kendra for current charges, accepted documents and the correct district approval route.

RajasthanPehchan / e-Mitra registration amendment

Check online scope; Registrar may be required

Step-by-step procedure

  1. Identify the registration area and record number and sign in through Rajasthan Pehchan / the linked e-Mitra SSO route.
  2. Pehchan’s official public information says the portal supports amendments to registered birth/death entries and language changes, but do not choose the menu for “change pending application” when you need a registered death amended.
  3. Prepare a change statement and provide documentary evidence to the jurisdictional Registrar; for an error in substance Rule 11(4) requires a declaration of true facts made by two credible persons.
  4. File through the appropriate Pehchan/e-Mitra amendment workflow or the Registrar as directed; where instructed, present the generated application print and originals for verification.
  5. Follow status through the official portal and download the fresh certificate after the Registrar approves the change.

Documents and proof: Existing death registration, proof of the true fact and, for substantive errors, the two-credible-person declaration under the Rajasthan rule.

Important limit: Rajasthan Pehchan distinguishes registration amendments from editing a pending online application. Read Rule 11 in the latest Rajasthan RBD rules through the Pehchan portal; deep document links can change. The e-Mitra applicant-instructions page concerns NEW registrations, so it is intentionally not linked as a correction application.

SikkimIssuing Registrar; Gangtok verification is a separate service

Registrar / manual confirmation

Step-by-step procedure

  1. Use the issuing registration area and Registrar printed on the old certificate rather than starting at a general Gangtok verification desk.
  2. Gangtok district publishes a “Verification for Issue of Death Certificates” counter process, but that confirms a document; it is not a verified amendment application.
  3. Ask the actual birth/death Registrar for the Section 15 correction route, showing the incorrect and correct data and where the hospital/local authority reported it.
  4. Submit the original record and supporting evidence; obtain the office’s exact list of any recommendations, witness declarations and verification steps needed for a CHANGE.
  5. Retain receipt and obtain a corrected Registrar-issued extract, not just a verification report.

Documents and proof: Original death registration, documents supporting changed data and any local Registrar-required statement.

Important limit: The Gangtok District “Verification for Issue” page is not a death correction SOP. Statewide online death-correction instructions were not verified.

Tamil NaduJurisdictional Registrar only; Chennai published SOP

Registrar / manual confirmation

Step-by-step procedure

  1. Check the registration jurisdiction. Tamil Nadu’s CRS FAQ states only the concerned Registrar can correct a registered entry; the public crstn.org death search/download screen is not a correction service.
  2. For a rural death, ask the applicable local Registrar identified by TN CRS (for example, the Village Administrative Officer in a village); for Chennai, contact Greater Chennai Corporation’s relevant birth/death Registrar.
  3. File a written application with the certificate and the proof of wrong-versus-correct details; for substantive mistakes, Tamil Nadu Rule 11(4) provides for declaration of true facts by TWO credible persons.
  4. In Chennai, the Corporation’s SOP describes document scrutiny and field verification if necessary, followed by amendment of the original register and certificate.
  5. Download the corrected Chennai certificate after approval or receive it through your own local body; older pre-2018 records may need local-body search.

Documents and proof: Certificate and supporting records, two credible-person declaration for substantial errors under Rule 11(4), plus source-institution confirmation when relevant.

Important limit: Chennai’s SOP applies to the Chennai Corporation; Tamil Nadu CRS says public downloads are generally for records from 1 January 2018. Outside Chennai use the jurisdictional Registrar.

TelanganaGHMC and CDMA correction services via MeeSeva

Online / assisted correction listed

Step-by-step procedure

  1. Check whether the registered death falls in GHMC Hyderabad or another municipal/CDMA jurisdiction; Telangana MeeSeva lists “Death Certificate Corrections – GHMC” separately from “Corrections in Death Certificate – CDMA”.
  2. For GHMC use the birth/death correction declaration issued on the Telangana government site; for other CDMA municipalities use the dedicated “CDMA Death Corrections” form.
  3. Obtain the registration unit ID, registration number/year, specify each field needing correction (name, date, father/mother, place or address) and provide informant details.
  4. For GHMC name corrections, published instructions list a nearest-relative declaration, two Gazetted Officer attestations, notarised affidavit, earlier certificates, supporting evidence and hospital consent; police/FIR/post-mortem records may be needed for medico-legal deaths.
  5. Submit through the appropriate MeeSeva service/centre, keep the transaction receipt, respond to Registrar verification and obtain the amended extract after approval.

Documents and proof: Telangana official GHMC declaration and CDMA correction forms; identity/older documentary proofs, original certificate and source hospital/police documentation as applicable.

Important limit: GHMC and CDMA forms are NOT interchangeable. Some government PDFs are older; document requirements and actual service access must be confirmed at the relevant local body.

TripuraMunicipal/urban authority; documented self-attestation policy

Registrar / manual confirmation

Step-by-step procedure

  1. Locate the issuing urban local body or rural Registrar on the certificate. Tripura government documents explicitly recognise “Correction of Birth/Death certificate” among Urban Development services.
  2. Submit a change application to that authority, identifying the death-registration number and the field entered incorrectly; do not use the e-District fresh death certificate menu unless the office confirms a correction option.
  3. Prepare documentary proof of the true particulars. A Tripura government 2015 memorandum includes correction of birth/death certificates among services for acceptance of self-attested documents, subject to departmental instructions.
  4. Ask the municipal office whether originals must be shown for verification and obtain a date-stamped receipt.
  5. Collect the signed decision and revised certificate; for a refusal use the Registrar appeal route.

Documents and proof: Certificate, applicant ID and field-specific self-attested records; local urban body may require additional formats.

Important limit: The Tripura memorandum establishes a self-attestation policy and the existence of a correction service, but not a complete statewide online correction process or current fee schedule.

Uttar PradeshPlace-of-occurrence Registrar; issuance portals differ

Registrar / manual confirmation

Step-by-step procedure

  1. Identify the authority on the certificate—municipal corporation, nagar palika, nagar panchayat or rural/health registration unit—using the place where death was recorded, not the family’s current city.
  2. The state health portal offers issue of death certificates, but a consistent public UP-wide correction workflow was not located; request Section 15 correction from the holder of the original register.
  3. Submit a written difference statement, entry/registration number and the documentary basis for the requested amendment; seek hospital source clarification if the institution reported incorrect data.
  4. Ask the Registrar which UP rule/form applies to the error, and whether the file needs municipal registrar or district verification.
  5. Keep the acknowledgement and request the corrected extract or a written refusal for appeal.

Documents and proof: Old certificate/registration number, field-specific evidence, applicant ID; additional declarations are determined by the appropriate registrar.

Important limit: UP health/issuance pages and citizen grievance sites are NOT dedicated death-correction portals. Statewide price and service deadline were not verified.

UttarakhandLocal Registrar; official circular on register errors

Registrar / manual confirmation

Step-by-step procedure

  1. Check the original municipal, rural or institutional Registrar shown on the record.
  2. Uttarakhand’s Medical Health Department publishes a circular titled “Correction of errors in Birth and Death Registers” (30 July 2003); use the circular index as a reference, not as an application portal.
  3. Write a correction request identifying the registration entry, the mistaken field and the evidence supporting the correct data.
  4. Take the request and supporting records to the concerned Registrar; ask whether the state circular imposes a particular approval, register retrieval or declaration process for that error.
  5. Obtain acknowledgement and updated certificate, or request the order with reasons to support an appeal.

Documents and proof: Certificate/entry and source documentation (identity, hospital report or family record) specific to the wrong field.

Important limit: The linked source is an official circular INDEX with a correction entry; the exact circular text, online form and state-wide correction deadline were not independently established.

West BengalJanma-Mrityu Tathya: Death Certificate Correction

Online / assisted correction listed

Step-by-step procedure

  1. Open West Bengal’s official Janma-Mrityu Tathya portal and go to Citizen Services → Death → “Death Certificate Correction” (not Download Certificate or Delayed Death Registration).
  2. Sign in using the portal’s required credentials; supply the registration/acknowledgement details and select the required death record.
  3. Provide the corrected field values and supporting proof as prompted by the live application; where the original institution supplied wrong facts, obtain an authoritative confirmation if requested.
  4. Submit the correction, preserve the acknowledgement and use “Track Application” rather than re-registering the death.
  5. After Registrar approval, use “Download Certificate” and “Death Certificate Verify” to confirm the corrected details.

Documents and proof: Existing certificate or number, applicant identification and field-specific evidence; the live screen determines additional uploads.

Important limit: The portal explicitly says correction services are operational, but its downloadable citizen manual did not load during review. Do not invent field menus, fees or processing times.

Andaman and Nicobar IslandsIsland / district record-holding Registrar

Registrar / manual confirmation

Step-by-step procedure

  1. Look up the original registration district and island on the death certificate; do not assume a single Port Blair service covers all island registration units.
  2. Use the district certificate directory to find the responsible office and request the existing death register record.
  3. Write an application under Section 15 explaining the incorrect versus correct particulars; provide records supporting the requested change.
  4. Ask the holding Registrar whether submissions require attendance, register retrieval or any island/district-level verification; do not apply through an issuance-only form.
  5. Take an acknowledgement and collect the updated entry extract from the same authority once approved.

Documents and proof: Old certificate/registration number, evidence of true entry and applicant ID.

Important limit: The official linked district certificate page identifies certificate services, not a UT-wide correction checklist or online correction facility.

ChandigarhBirth & Death Registrar counter; published 14-day correction

Online / assisted correction listed

Step-by-step procedure

  1. Visit the Birth & Death counter with the death certificate; the official Chandigarh page says the clerk checks the data in the registration computer.
  2. For a MINOR spelling error, file a correction request with documentary proof and pay the applicable counter fee; receive the receipt and collection date.
  3. For a MAJOR change (the site names deceased’s father, mother or address), submit affidavit, statements by two credible people, institutional report and documentary proof as required.
  4. The dealing staff checks the register and sends the case to the Registrar for approval; approved corrections are entered in and signed against the underlying record.
  5. The data-entry operator prepares a fresh certificate for distribution. Chandigarh publishes a 14-day correction schedule; keep the receipt for follow-up.

Documents and proof: Minor correction: application and proof. Major correction: affidavit, two credible-person statements, institute report and additional documentary evidence, as the Chandigarh page expressly specifies.

Important limit: The 14-day timeline belongs to the Chandigarh administration’s published service process, not other states/UTs.

Dadra and Nagar Haveli and Daman and DiuMunicipal Council or District Panchayat Registrar

Registrar / manual confirmation

Step-by-step procedure

  1. Identify which area holds the registration—Daman, Diu or Dadra and Nagar Haveli—and whether the issuing authority was a Municipal Council or District Panchayat.
  2. The UT government directory lists municipal “Issuance of Birth & Death Certificate” and Panchayat “Registration of Death”; neither listing is a verified correction web form.
  3. Approach the exact issuing Registrar and request correction of the existing record under Section 15, rather than selecting a fresh issuance request.
  4. Attach the certificate, a written before/after comparison and proof of the disputed fact. Ask about local verification/approval and original-register custody.
  5. Preserve the receipt and collect an authorised corrected extract; use the district Registrar if the application is refused.

Documents and proof: Original certificate, registration particulars and evidence of the correct entry; local office confirms additional formalities.

Important limit: Official UT directories identify multiple service authorities but not a single combined UT-wide correction process, fee or online form.

DelhiMCD RBD portal / local registration office

Check online scope; Registrar may be required

Step-by-step procedure

  1. Check whether the registration belongs to Municipal Corporation of Delhi, New Delhi Municipal Council or Delhi Cantonment. MCD’s online process must not be assumed to apply to NDMC/Cantonment.
  2. For MCD, use the consolidated RBD portal and the Citizen Login route. The MCD site says its former EDMC, NDMC and SDMC municipal records have been unified, and some certificates have new registration numbers.
  3. Locate the existing certificate/registration number, then ask for the record CORRECTION action in the live portal or at the assigned MCD registration office; use supporting evidence of the specific wrong field.
  4. Follow the current MCD citizen user manual and department verification process, retaining the acknowledgement. Do not use the older city SOP as proof that a particular button still exists unchanged.
  5. Download the newest approved certificate and verify it against the current MCD number. For NDMC or Cantonment registrations, contact their own Registrar.

Documents and proof: Existing death registration, applicant and deceased identifying information, documentary proof of change; current MCD flow determines extra evidence.

Important limit: Newer MCD portal warns of renumbering after unification. Some older “NDMC” legacy URL labels refer to former North Delhi Municipal Corporation, not today’s New Delhi Municipal Council.

Jammu and KashmirJanSugam listed but correction service under modification

Online correction unavailable

Step-by-step procedure

  1. Identify the registering urban local body or rural Registrar. Official J&K district directories name the service “Correction in the Existing Death Record and Issuance of Certificate”.
  2. The corresponding JanSugam service page currently says “Service Under Modification”; DO NOT treat the link as a working online application or submit confidential paperwork there.
  3. Take the original death entry and a written list of changes with supporting proof to the issuing Registrar or relevant urban-local-body office; ask which interim manual route is operating while JanSugam changes are underway.
  4. Get the office’s document checklist and stamped acknowledgement rather than relying on an inaccessible upload process.
  5. Obtain the amended certificate from the competent authority, or a written refusal and the district appeal details.

Documents and proof: Existing certificate/record number, applicant ID and documentary evidence; manual submission requirements must be confirmed with the concerned Registrar.

Important limit: Online correction application status: “SERVICE UNDER MODIFICATION” as checked 31 October 2026. The page should be treated as SERVICE-STATUS EVIDENCE ONLY, not a working apply link.

LadakhLeh / Kargil record-holding Registrar

Registrar / manual confirmation

Step-by-step procedure

  1. Locate the original registration area within Leh or Kargil and identify the issuing office on the certificate.
  2. Ladakh’s official certificate page links to CRS for issuance; it does not give a verified online correction application for old or new death entries.
  3. Submit a written Section 15 correction request to that Registrar, with both the recorded and desired particulars and supporting proof.
  4. Ask the local office whether it can update its electronic record directly or must examine a paper register; obtain the required declaration and verification instructions from them.
  5. Collect the revised certified extract or written decision. Do not use Jammu and Kashmir’s JanSugam service for a Ladakh registration.

Documents and proof: Old registration/certificate, identity or hospital evidence, applicant ID and any local office declarations.

Important limit: Ladakh is a separate UT; the linked “application for issuance” page is an authority/CRS reference, NOT proof of an available death-correction service.

LakshadweepIsland registration unit; Registrar / CRS

Registrar / manual confirmation

Step-by-step procedure

  1. Identify the specific island and registration unit that recorded the death; the family’s present mainland location does not change jurisdiction.
  2. Request a register search and the existing entry details from that local Registrar, whether held electronically or in an earlier physical register.
  3. Provide a written Section 15 request with the mistaken and correct data and documentary proof of the change.
  4. Ask the island registration office for its current method of filing, whether attendance or certified copies are needed, and how a Registrar approves an old-record change.
  5. Retain the acknowledgement and request the corrected certified extract.

Documents and proof: Old certificate/registration particulars, field-specific documentary proof and applicant identification.

Important limit: The general national CRS portal is not a Lakshadweep correction application. A specific UT checklist, fee and service timeline could not be verified.

PuducherryPondicherry Municipality État Civil; separate outlying districts

Check online scope; Registrar may be required

Step-by-step procedure

  1. Identify whether the record was made in Puducherry town, Karaikal, Mahe or Yanam; each original registering authority matters.
  2. For a Pondicherry Municipality entry, its Birth & Death (État Civil) section explicitly lists “Application for Correction in Birth / Death Certificate” and “Correction fee for Birth/Death Certificate”.
  3. Approach the municipality’s Registrar to obtain the current correction application; enter the registration reference, present error, desired correction and relationship to the deceased.
  4. Attach evidence supporting the requested particulars and submit with the applicable fee/receipt; the published page lists ₹1 for an application and ₹10 as a correction fee, but confirm current payment at the counter.
  5. After Registrar scrutiny, collect the amended certificate. For Karaikal, Mahe or Yanam obtain the local registration office’s instructions rather than using Puducherry municipality’s pricing or form.

Documents and proof: Original certificate, registration reference and proof of corrected fact; the municipal website does not give an exhaustive correction enclosure list.

Important limit: The linked municipal service is specific to Pondicherry Municipality and does NOT automatically cover all parts of the UT. Distinguish correction charges from charges for fresh extracts.

Before opening a government link

Identify the kind of correction

A

Wrong name or spelling

Compare the certificate with the original death report and older identity records. A typographical difference may have a simpler Registrar enquiry than a disputed change of identity.

B

Wrong date or place of death

Check the reporting institution, Form 2 and relevant hospital or police records. The Registrar may need to verify the source of the registered event before deciding.

C

Wrong parent, spouse or age

Show the basis for the correct relationship or age on the date of death. State rules may require witness declarations for substantive errors.

D

Old certificate missing online

Ask the issuing unit to search its paper register. Digitisation, certificate re-issue, cancellation and correction are separate services.

A change to a pending application is not necessarily a Section 15 correction of a final registered entry. Death registration, the medical certificate of cause of death (MCCD Form 4/4A), and a cremation certificate are different records with different issuing authorities.

Prepare the request

Steps and supporting documents

  1. Locate the existing entry.Note the issuing authority, registration number, year and date of death.
  2. Write the requested changes.List “currently recorded” and “correct particulars” separately for every field.
  3. Attach evidence and apply.Use the local correction service or submit to the Registrar. Keep the acknowledgement.
  4. Complete verification.Provide institutional confirmation, declarations or other records if required.
  5. Check the revised certificate.Check English and local-language spellings, dates and relationship fields before using it.

Prepare these records

  • Existing death certificate and applicant identification.
  • Accepted identity or education records for a name or age correction.
  • Relevant relationship records for a parent’s or spouse’s name.
  • Hospital confirmation for an error in the institutional report; police/post-mortem records where applicable.

This is a preparation list, not a nationwide mandatory checklist. Age evidence must establish age on the date of death. Documents accepted by one authority may not be accepted by another.

Common questions

Before you submit

Can I change a death certificate after it has already been issued?
Yes, Section 15 allows a Registrar to correct an existing registered entry after the required proof and state-rule conditions are met. The Registrar decides the case; a portal that edits a pending application is not automatically the same process.
Can a family member correct the hospital’s MCCD Form 4/4A through a municipal portal?
No. The medical certificate of cause of death and the civil death registration certificate are different records. Ask the issuing medical institution about an error in its medical document and the Registrar about any resulting error in the civil register.
Can I correct a death certificate online?
Some authorities provide a dedicated correction service. A portal that only issues or downloads certificates may not accept corrections. Check the state guide and the Registrar holding your record.
Is an affidavit compulsory for every spelling mistake?
No single affidavit requirement applies to every correction across India. The error type and local rules determine whether declarations, witnesses or an affidavit are needed.
Should I register the death again to correct a mistake?
No. Request correction of the existing entry. Delayed registration is a different procedure for an event that was not registered.
Can I correct a certificate from another state?
Start with the authority holding the existing death record, normally in the place where the death occurred. Your current residence does not transfer that record to a different Registrar.
What if the old certificate is not online?
Ask the record custodian to locate the original registration. Legacy digitisation and correction are separate tasks; an absent online result alone does not establish that the death was never registered.
How long does a correction take?
There is no single nationwide period. Chandigarh publishes 14 days; Solapur lists 31 days. These are local service periods, not promises for other authorities.

If the correction is refused

Ask for the written decision and reasons. Section 25A provides an appeal from the Registrar to the District Registrar, and from the District Registrar to the Chief Registrar, within 30 days of the action or receipt of the order. Use the prescribed form and manner. The appellate authority must decide within 90 days of the appeal.

Legal basis and official references

Reference labels distinguish live citizen service pages, staff manuals, forms, rulebooks and issuance-only directories. We have not described a general government landing page as a working correction form. A portal may be temporarily inaccessible or change after review.

Section 15 permits correction of an erroneous entry, subject to state rules, while preserving the original entry. Clerical and substantive errors can require different evidence. There is no single affidavit, fee or correction timeline established for every Indian authority.

Local sources are linked within each state entry. Reviewed 31 October 2026. Where a correction checklist was not publicly available, confirm it with the record-holding authority before applying.
